ABSTRACT

Abstract Objective Advances in medicine have increased the life expectancy of pediatric patients with chronic illnesses, and challenges with the guided transition of adolescents and young adults from pediatric clinics to adult clinics have grown. The aim of this study was to better understand readiness and factors related to this transition process in Brazil. Method In this cross-sectional study of 308 patients aged from 16 to 21 years under follow-up in pediatric specialties, the degree of readiness for transition was assessed using the Transition Readiness Assessment Questionnaire (TRAQ) and its domains. Associations with demographic data, clinical data, socio-economic level, medication adherence, family functionality, and parental satisfaction with health care were evaluated. Results The median TRAQ score was 3.7 (3.2 - 4.2). Better readiness was associated with female patients, socio-economic class A-B, current active employment, higher level of education, not failing any school year, attending medical appointments alone, functional family, and a good knowledge of disease and medications. A low correlation was observed between TRAQ and age. TRAQ presented good internal consistency (alpha-Cronbach 0.86). In the multiple linear regression, TRAQ score showed a significant association with female gender, advanced age, socio-economic class A-B, better knowledge of disease and medications, and independence to attend appointments alone. Conclusion TRAQ instrument can guide healthcare professionals to identify specific areas of approach, in order to support adolescents with chronic disease to set goals for their own personal development and improve their readiness to enter into the adult healthcare system. In this study, some factors were related to better TRAQ scores.

ABSTRACT

Introduction: COVID-19 unfolded differently in Pará and Rio Grande do Sul, Brazil, owing to distinct socioeconomic contexts. From 2020 to 2022, both states implemented diverse measures against the virus SARS-CoV-2, including vaccination and variant monitoring, tailored to their specific challenges. Understanding regional impacts on COVID-19 indicators is crucial for designing effective control strategies. Objective: to analyze the incidence, mortality, and lethality of COVID-19 in Pará and Rio Grande do Sul and the trends of these indicators from 2020 to 2022. Methods: ecological study with time series from public and official data available in the Health Secretariat of Pará and Rio Grande do Sul, including all cases and deaths by COVID-19 from February 2020 to December 2022. Lethality, mortality, and incidence rates were calculated. Prais-Winsten regression analysis was used, and trends were classified as stationary, increasing, or decreasing. Significant differences were considered when the p-value is <0.05. Results: when examining the lethality rates between the states of Para and Rio Grande do Sul, an observable trend emerged during the analyzed period. It became evident that the total lethality rate consistently remained higher in Para. Noteworthy peaks in lethality were mainly observed during the months of April 2020, May 2020, and March 2021. The incidence rates showed increasing trends during 2020, both in Pará with a daily percentage change (DPC) of 1.69% (p <0.05) and in Rio Grande do Sul with a DPC of 1.70% (p<0.05). In 2021, the incidence was decreasing (p <0.05) in both states, with a DPC of 0.60% in Pará and 0.64% in Rio Grande do Sul and continued this trend in Pará in 2022 (DPC of -0.50% p <0.05), becoming stationary in Rio Grande do Sul, with a non-significant p-value (p> 0.05).Conclusion: the positive impact of the vaccination program is reflected in the evolution of the pandemic. During the study period Rio Grande do Sul and Para exhibited a stationary incidence trend, emphasizing the need for continued monitoring of cases and morbidity across various age and demographic groups.

Introduction: Down syndrome is the most common genetic cause of intellectual disability, and the use of augmented reality can be improving the cognitive, motor, and literacy skills of this population Objective: to compare statistical differences in learning between individuals with Down syndrome and individuals with typical development using augmented reality games.Methods: we compared the reaction time before and after the virtual reality tasks, in addition to the performance in these tasks, which consisted of correctly identifying numbers and letters in 46 people with Down syndrome and 46 controls with typical development.Results: our results indicate that the total points for the typical development group were higher (M = 13.0 and 11.9) when compared to the Down syndrome group (M = 6.6 and 4.6) for letters of the alphabet and numbers, respectively. Furthermore, the results indicated that participants in both groups were more accurate in identifying alphabetic symbols when compared to numerical symbols, and both groups were sensitive to the number of symbols presented in each phase. The down syndrome group had a lower performance when compared to the typical development group.Conclusion: despite the need for further studies, our results support the outcome that there is clinical utility of an intervention based on virtual reality tasks for people with Down syndrome. In conclusion, the use of this technology to improve the reaction time of this population is considered useful


Introdução: a síndrome de Down é uma das principais doenças genéticas que causam deficiência intelectual, e o uso da realidade aumentada pode aumentar as habilidades cognitivas, motoras e de aprendizado dessa população. Objetivo: comparar diferenças estatísticas na aprendizagem entre indivíduos com síndrome de Down e indivíduos com desenvolvimento típico usando jogos de realidade aumentada.Métodos: comparamos o tempo de reação antes e depois das tarefas de realidade virtual, além do desempenho nessas tarefas, que consistiam em identificar corretamente números e letras em 46 pessoas com síndrome de Down e 46 controles com desenvolvimento típico.Resultados: nossos resultados indicam que o total de pontos para o grupo de desenvolvimento típico foi maior (M = 13,0 e 11,9) quando comparado ao grupo com síndrome de Down (M = 6,6 e 4,6) para letras do alfabeto e números, respectivamente. Além disso, os resultados indicaram que os participantes de ambos os grupos foram mais precisos na identificação de símbolos alfabéticos quando comparados aos símbolos numéricos, e ambos os grupos foram sensíveis ao número de símbolos apresentados em cada fase. O grupo com síndrome de Down teve desempenho inferior quando comparado ao grupo de desenvolvimento típico.Conclusão: apesar da necessidade de mais estudos, nossos resultados suportam a utilidade clínica de uma intervenção baseada em tarefas de realidade virtual para pessoas com síndrome de Down. Em conclusão, considera-se útil o uso dessa tecnologia para melhorar o tempo de reação dessa população

ABSTRACT Objective: This study aims to investigate the ability of the six-minute walk distance (6MWD) as a prognostic marker for midterm clinical outcomes three months after coronary artery bypass grafting (CABG), to identify possible predictors of fall in 6MWD in the early postoperative period, and to establish the percentage fall in early postoperative 6MWD, considering the preoperative baseline as 100%. Methods: A prospective cohort of patients undergoing elective CABG were included. The percentage fall in 6MWD was assessed by the difference between preoperative and postoperative day (POD) five. Clinical outcomes were evaluated three months after hospital discharge. Results: There was a significant decrease in 6MWD on POD5 compared with preoperative baseline values (percentage fall of 32.5±16.5%, P<0.0001). Linear regression analysis showed an independent association of the percentage fall of 6MWD with cardiopulmonary bypass (CPB) and preoperative inspiratory muscle strength. Receiver operating characteristic curve analysis revealed that the best cutoff value of percentage fall in 6MWD to predict poorer clinical outcomes at three months was 34.6% (area under the curve = 0.82, sensitivity = 78.95%, specificity = 76.19%, P=0.0001). Conclusion: This study indicates that a cutoff value of 34.6% in percentage fall of 6MWD on POD5 was able to predict poorer clinical outcomes at three months of follow-up after CABG. Use of CPB and preoperative inspiratory muscle strength were independent predictors of percentage fall of 6MWD in the postoperative period. These findings further support the clinical application of 6MWD and propose an inpatient preventive strategy to guide clinical management over time.

ABSTRACT

Este artigo propõe-se a apresentar aspectos essenciais das análises construídas a partir de uma pesquisa teórica que teve o objetivo de analisar as contribuições que a Pedagogia Histórico-crítica e a Psicologia Histórico-cultural apresentam em relação às potencialidades e possibilidades de que o ensino escolar e a atividade de ensino do professor promovam o desenvolvimento da atenção voluntária dos estudantes. Dessa forma, assumiu-se como objeto de estudo o desenvolvimento dessa função psicológica em sua dimensão cultural. As análises buscaram mostrar, nas produções da Psicologia Histórico-cultural e da Pedagogia Histórico-crítica, elementos acerca do desenvolvimento da atenção que possam auxiliar na concretização do objetivo apresentado por esta investigação, destacando a necessidade de considerar o materialismo histórico-dialético como perspectiva teórico-metodológica orientadora das teorias analisadas e também das análises propostas por este trabalho. O material empírico da investigação consistiu em artigos, teses e livros que discorrem sobre o desenvolvimento da atenção e o papel do ensino para tal desenvolvimento. Como síntese das dimensões essenciais identificadas por meio da análise das obras destaca-se a necessidade de compreender que o desenvolvimento das funções psicológicas humanas não se esgota em um processo de maturação biológica, mas articula e submete esta dimensão aos aspectos culturais, históricos e sociais do desenvolvimento humano, sendo que a adoção da Psicologia Histórico-cultural, como perspectiva teórico-metodológica para a organização do ensino e da atividade docente, mostra ser uma importante ferramenta para o enfrentamento da lógica medicalizante. This article proposes to present essential aspects of the analyzes constructed from a theoretical research that had the objective of analyzing the contributions that Historical-Critical Pedagogy and Historical-Cultural Psychology present in relation to the potentialities and possibilities that the school education and the teaching activity promote the development of voluntary attention of the students. Thus, the development of this psychological function in its cultural dimension was assumed as the object of study. The analyzes sought to highlight elements of the development of attention in the productions of Historical-Cultural Psychology and of Historical-Critical Pedagogy that may assist in the achievement of the objective presented by this research, highlighting the need to consider historical-dialectical materialism as a theoretical perspective -methodological guide of the theories analyzed. The empirical material of the research consisted of articles, theses and books that discuss the development of attention and the role of teaching for such development. As a synthesis of the essential dimensions identified through the analysis of the works it is necessary to understand that the development of human psychological functions is not exclusively in a process of biological maturation, but rather it articulates and submits this dimension to the cultural, historical and social aspects of human development. The adoption of Historical-Cultural Psychology as a theoretical-methodological perspective for the organization of teaching and teaching activity, proves to be an important tool for confronting the medical logic. Este artículo presenta los aspectos esenciales de las discusiones realizadas a partir de una investigación teórica que tuvo el objetivo de analizar las contribuciones que la Pedagogía Histórico-Crítica y la Psicología Histórico-Cultural direccionan a la enseñanza escolar y la actividad del profesor en la promoción del desarrollo de la atención voluntaria de los estudiantes. Así, se asumió como objeto de estudio el desarrollo de esa función psicológica en su dimensión cultural. Los análisis buscaron evidenciar, en las producciones de la Psicología Histórico-Cultural y de la Pedagogía HistóricoCrítica, elementos acerca del desarrollo de la atención que puedan auxiliar en la concreción del objetivo presentado por esta investigación, destacando la necesidad de considerar el materialismo histórico-dialéctico como perspectiva teórica-metodológica orientadora de las teorías analizadas y también de los análisis propuestos por este trabajo. El material empírico de la investigación consistió en artículos, tesis y libros que discurren sobre el desarrollo de la atención y el papel de la enseñanza para eso proceso. Como síntesis de las dimensiones esenciales identificadas por medio del análisis de las obras se destaca la necesidad de comprender que el desarrollo de las funciones psicológicas humanas no se agota en un proceso de maduración biológica sino que articula y somete esta dimensión a los aspectos culturales, históricos y sociales del desarrollo humano, siendo que la adopción de la Psicología Histórico-Cultural, como perspectiva teóricometodológica para la organización de la enseñanza y la actividad docente, demuestra ser una importante herramienta para el enfrentamiento de la lógica de la medicalización de la educació. ABSTRACT Background: Stroke is a serious complication of sickle cell anemia (SCA). The transcranial Doppler (TCD) is the risk-screening tool for ischemic strokes. The objective of the study was to describe the clinical progression of children with SCA who presented with high risk for stroke by TCD or relevant changes by magnetic resonance angiography (MRA) and underwent the regular transfusion program (RTP) and/or hydroxyurea (HU) treatment between 2007 and 2018. Method: This was a neonatal retrospective/prospective cohort study with children born between 1999 and 2014 with the homozygotic form (HbSS) or Sβ0-thalassemia who underwent TCD at least once. Results: Of the 718 children screened during this period, 675 had HbSS and 43 Sβ0-thalassemia. In 54 children (7.5%), all with HbSS, a high-risk TCD (n = 45) or, when the TCD was inconclusive, an MRA with cerebral vasculopathy (n = 9) was used for detection. Of these, 51 started the RTP and the families of three refused treatment. Of the 43 children with a highrisk TCD who initiated the RTP, 29 (67.4%) reverted to low risk. In 18 of them (62%), HU was started at the maximum tolerated dose (MTD) before transfusion discontinuation. None of these 29 patients had a stroke. Eight children (18.6%) maintained a high-risk TCD, even using the RTP/HU and two had a stroke. Conclusions: The TCD was confirmed as a viable tool for tracking patients with a risk for stroke. The RTP was effective in preventing the primary event. New strategies are necessary to prevent stroke using HU and new drugs, in addition to bone marrow transplantation.

ABSTRACT

INTRODUCTION: the growth of coronavirus indices in the North region highlights the region's historical social inequalities and the problems in accessing citizenship. OBJECTIVE: to analyze the mortality and lethality of COVID-19 in the state of Pará, Brazil. METHODS: this is an ecological study with a time series design of secondary data. All registered cases and deaths reported by COVID-19 in the period from March 2020 to June 2021, in the state of Pará, Brazil, were considered. The incidence and mortality and lethality rates were used. The daily percentage variation and their respective 95% confidence intervals were estimated. RESULTS: the total number of confirmed cases and deaths from COVID-19 in the state of Pará was 552,937 and 15,469, respectively, from March 2020 to June 2021. Incidence and mortality rates from March 2020 to June 2021 were, respectively, 6,407.9 and 179.3 per 100,000 inhabitants and the lethality was 43.3. Regarding the analysis of the daily trend of rates in the period from March 2020 to June 2021, both mortality and incidence increased. CONCLUSION: it was found that the behavior of the trend of rates in the first wave was increasing in the incidence of confirmed cases and the lethality decreasing, and in the second wave, the mortality and lethality rates were increasing.


INTRODUÇÃO: o crescimento dos índices do coronavírus na região Norte evidencia as desigualdades sociais históricas da região e os problemas no acesso à cidadania. OBJETIVO: analisar a mortalidade e letalidade da COVID-19 no estado do Pará, Brasil. MÉTODO: trata-se de um estudo ecológico com delineamento de série temporal de dados secundários. Foram consideradas todos os casos registrados e óbitos notificados por COVID-19 no período de março de 2020 a junho de 2021, no estado do Pará, Brasil. Foi utilizado a taxa de incidência, mortalidade e letalidade. Estimou-se a variação percentual diário e seus respectivos intervalos de 95% de confiança. RESULTADO: o total de casos confirmados e óbitos por COVID-19 no estado do Pará foi de 552.937 e 15.469, respectivamente, no período de março de 2020 a junho de 2021. As taxas de incidência e mortalidade do período de março de 2020 a junho de 2021 foram, respectivamente, 6.407,9 e 179,3 por 100.000 habitantes e a letalidade foi 43,3. Com relação à análise de tendência diária das taxas no período de março de 2020 a junho de 2021 tanto a mortalidade quanto a incidência foram crescentes. CONCLUSÃO: constatou-se que o comportamento da tendência das taxas na primeira onda foi crescente na incidência de casos confirmados e a letalidade decrescente e, na segunda onda, as taxas de mortalidade e letalidade foram crescentes.

INTRODUCTION: COVID-19 impacted health systems worldwide; the virus quickly spread in Brazil, reaching its 27 Federative units peculiarly. The northern country region recorded the lowest number of cases and accumulated deaths from the disease. However, it is a region of sizeable territorial extension and low demographic density, marked by socioeconomic inequalities and vulnerable groups, such as indigenous tribes, riverine peoples, and quilombolas. Sociodemographic factors may contribute to the dissemination of the coronavirus in this territory; thus, studies are needed to analyze the epidemiological indicators related to the pandemic. OBJECTIVE: to evaluate incidence, mortality, and case fatality of COVID-19 trends in the state of Amapá, Brazil, from March 2020 to April 2021. METHODS: an ecological time-series study was conducted with publicly accessible data from the Health Department of the State of Amapá. Incidence and mortality rates per 100,000 inhabitants and percentage case fatality were calculated. Crude rates were calculated by municipalities, age, and sex, per month. The Prais-Winsten regression test was performed, and the trends of monthly rates were classified as increasing, decreasing, or flat. RESULTS: during the study period, there were 99.936 cases and 1,468 deaths accumulated by COVID-19 in the State of Amapá, Brazil. Macapá and Santana's cities, which have the highest demographic density and Human Development Index (HDI), had the highest number of cases and deaths. The most vulnerable population was elderly males aged 70 years or over; these individuals had the highest cumulative incidence, case fatality, and mortality rates. The second wave of the disease (October 2020 to April 2021) illustrated a more aggravating scenario, with increasing incidence and mortality rates. CONCLUSION: the COVID-19 pandemic in the state of Amapá, Brazil, is in increasing evolution, which illustrates that non-pharmacological prevention measures and acceleration of vaccination must be strengthened to avoid the development of future waves of the disease.


INTRODUÇÃO: a COVID-19 impactou os sistemas de saúde em todo o mundo, rapidamente o vírus disseminou-se no Brasil, atingindo de modo distinto as 27 unidades Federativas do país. A região norte do Brasil registrou o menor número de casos e óbitos acumulados da doença. Entretanto, trata-se de região de grande extensão territorial e baixa densidade demográfica, marcada por desigualdades socioeconômicas, presença de população vulnerável como tribos indígenas, povos ribeirinhos e quilombolas. Os fatores sociodemográficos podem contribuir para a disseminação do coronavírus na região, assim, fazem-se necessários estudos que analisem os indicadores epidemiológicos relacionados à pandemia. OBJETIVO: avaliar as tendências da incidência, mortalidade e letalidade da COVID-19 no estado do Amapá, durante o período de março de 2020 a abril de 2021. MÉTODO: foi realizado um estudo ecológico de séries temporais, com dados de livre acesso, oriundos da Secretaria de Saúde do Estado do Amapá. Foi calculado a taxa de incidência e mortalidade por 100.000 habitantes e letalidade percentual. As taxas brutas foram calculadas por municípios, idade e sexo e por mês. Foi realizado o teste de regressão de Prais-Winsten, as tendências das taxas mensais foram classificadas em crescentes, decrescentes ou estacionárias. RESULTADOS: houve 99,936 casos e 1,468 óbitos acumulados por COVID-19 no Estado do Amapá durante o período estudado. As cidades de Macapá e Santana, que apresentaram densidades demográficas e Índice de Desenvolvimento Humano (IDH) mais elevados, apresentaram o maior número de casos e óbitos. A população mais vulnerável foi constituída pelos idosos do sexo masculino, com idade igual ou superior a 70 anos, estes indivíduos apresentaram as maiores taxas acumuladas de incidência, letalidade e mortalidade. A segunda onda da doença (outubro de 2020 a abril de 2021) ilustrou um cenário mais agravante, com crescentes nas taxas de incidência e mortalidade. CONCLUSÃO: a pandemia da COVID-19 no estado do Amapá está em crescente evolução, o que ilustra que medidas de prevenção não farmacológicas e aceleração à vacinação devem ser fortalecidas para evitar o desenvolvimento de futuras ondas da doença.

ABSTRACT

Este texto tem como objetivo desvelar as determinações que tornam a educação escolar uma das atividades humanas responsáveis por compor a dinâmica do processo de formação de valores orientadores das atitudes dos indivíduos. Iniciamos estabelecendo relações entre a sociedade, a educação escolar e o processo de formação dos indivíduos. Em seguida apresentamos características gerais da dinâmica de formação do caráter segundo a psicologia histórico-cultural ­ explicitado como produto da atividade concreta dos indivíduos ­ de modo a expor alguns determinantes que recaem sobre a formulação dos valores orientadores das condutas individuais. Neste processo, buscamos tornar evidente a inevitável incidência da educação escolar sobre a formação da conduta dos estudantes. Finalizamos com a análise de situações concretas observadas no cotidiano de uma escola de educação infantil e extraídas da tese de doutoramento de uma das autoras (Silva, 2017). Apresentamos, então, algumas perspectivas para a formulação intencional e sistematizada de práticas educativas em que se busque transmitir valoração positiva em relação aos interesses coletivos universais como condição para o efetivo desenvolvimento individual.


This paper aims to unveil the determinations that make school education one of the human activities which are responsible for creating the formation of values and dynamics that guide the attitudes of individuals. We started by establishing relationships between society, school education, and the formation of individuals. Then we present general characteristics of the character formation's dynamics according to historical-cultural psychology - explained as a product of the concrete activity of individuals - to expose some determinants that fall on the formulation of the guiding values of individual conduct. In this process, we seek to make evident the inevitable impact of school education on the formation of student's conduct. We conclude with the analysis of concrete situations observed in a field study on an early childhood school and extracted from the doctoral thesis of one of the authors (Silva, 2017). We present, then, some perspectives to corroborate intentionally and systematized educational practices towards the transmission of positive valuation related to universal collective interests as a condition for effective individual development.

ABSTRACT OBJECTIVE: To describe mortality due to hepatitis B and C as underlying cause in the municipality of São Paulo, verifying the trend of these rates, and to assess the association of these diseases with others, from 2002 to 2016. METHODS: This is a time series study on mortality due to hepatitis B and C according to sex, with data from the Sistema de Informação de Mortalidade (SIM - Mortality Information Sistem). Prais Winsten regression was used in rate trend analysis. RESULTS: The findings of this study showed a trend of decline of mortality from hepatitis B and C in recent years, particularly among males. These infections were important associated causes of liver cell carcinoma and HIV. The proportion of deaths under 70 years of age stands out. CONCLUSIONS: The study provides a baseline for research on mortality trend and the impact of interventions, given the history of expanded detection and supply of treatments, including the most recent antivirals in Brazil, since 2015.


RESUMO OBJETIVO: Descrever a mortalidade por hepatites B e C como causa básica no município de São Paulo, verificando a tendência dessas taxas, e avaliar a associação dessas doenças a outras no período de 2002 a 2016. MÉTODOS: Trata-se de um estudo de série temporal sobre mortalidade por hepatites B e C segundo sexo, com dados do Sistema de Informação de Mortalidade. A regressão de Prais Winsten foi usada na análise de tendência das taxas. RESULTADOS: Os achados do presente estudo mostraram tendência de declínio da mortalidade por hepatites B e C nos últimos anos, particularmente entre pessoas do sexo masculino. Essas infecções foram causas associadas importantes ao carcinoma de células hepáticas e ao HIV. Destaca-se a proporção de óbitos com menos de 70 anos de idade. CONCLUSÕES: O estudo fornece uma linha de base para pesquisas de tendência de mortalidade e de impacto de intervenções, visto o histórico de ampliação da detecção e oferta de tratamentos, incluindo os mais recentes antivirais no Brasil, desde 2015.

Resumo Objetivo: descrever a ocorrência da hepatite B entre gestantes, a realização de imunoprofilaxia e a transmissão vertical e perinatal nas crianças expostas ao vírus na rede de Atenção Primária à Saúde do estado de São Paulo, Brasil. Métodos: estudo transversal sobre registros de pré-natal de gestantes atendidas de janeiro a junho de 2012 e coorte dos recém-nascidos; foram descritas as frequências dos resultados e calculada a estimativa da ocorrência da hepatite B. Resultados: foram incluídas 6.233 gestantes, das quais 53,1% com 20 a 29 anos de idade, 58,7% com 8 a 11 anos de estudo, 53,3% brancas e 73,9% com companheiro; a ocorrência de hepatite B foi de 0,13% (IC95%: 0,04 a 0,21%); das oito crianças de mães com hepatite B crônica, seis tiveram esquema vacinal completo e não houve transmissão vertical ou perinatal. Conclusão: observou-se baixa ocorrência de hepatite B em gestantes e ausência de transmissão vertical ou perinatal.


Resumen Objetivo: describir el aparecimiento de hepatitis B en mujeres embarazadas, la inmunoprofilaxis y la transmisión vertical y perinatal en niños expuestos al virus en la red de atención primaria en el estado de São Paulo, Brasil. Métodos: estudio transversal con registros prenatales de mujeres embarazadas atendidas de enero a junio de 2012 y cohorte de recién nacidos; se describieron las frecuencias y se calculó el surgimiento de la hepatitis B. Resultados: se incluyeron 6.233 gestantes, de las cuales 53,1% con 20 a 29 años de edad, 58,7% con 8 a 11 años de estudios, 53,3% blancas y 73,9% viviendo en pareja; la ocurrencia de hepatitis B fue del 0,13% (IC95%: 0,04 to 0,21%); entre ocho hijos de madres con hepatitis B crónica, seis tenía un calendario de vacunación completo y no había transmisión vertical o perinatal. Conclusión: hubo baja ocurrencia de hepatitis B en mujeres embarazadas y ausencia de transmisión vertical o perinatal.


Abstract Objective: to describe the occurrence of hepatitis B among pregnant women, immunoprophylaxis and vertical and perinatal transmission in children exposed to the virus in the São Paulo state primary care network, Brazil. Methods: this was a cross-sectional study using prenatal records of pregnant women attending health services between January and June 2012 and a cohort of newborns; the frequencies of the results were described and the estimated occurrence of hepatitis B was calculated. Results: 6,233 pregnant women were included, of whom 53.1% were between 20-29 years old, 58.7% had 8-11 years of schooling, 53.3% were white, and 73.9% lived with a partner; occurrence of hepatitis B was 0.13% (95%CI 0.04; 0.21); of the eight children of mothers with chronic hepatitis B, six had a complete vaccination schedule, and there was no vertical or perinatal transmission. Conclusion: there was low occurrence of hepatitis B in pregnant women and absence of vertical or perinatal transmission.

ABSTRACT Objective To evaluate the effects of photodynamic therapy (PDT) on the dura mater using the photosensitizers aluminum chloride phthalocyanine and methylene blue in in vivo assays. Methods Fifty-six male Wistar rats were divided into two groups; one submitted to PDT and the other submitted to the photosensitizers without their photoactivation (control). The photosensitizers were applied to the dura mater after laminectomy at the T10 level. The methods used for assessment were the Basso, Beattie and Bresnahan (BBB) functional evaluation scale and study of the dura mater by light microscopy. Results No changes in motor activity were observed in the animals submitted to PDT compared to control. Histological and pathological evaluation did not show any differences between the group exposed to activated photosensitizers and the control group with regard to the inflammatory process and tissue necrosis. Conclusion The joint use of PDT with the photosensitizing pharmaceuticals aluminum chloride phthalocyanine and methylene blue did not induce any clinical neurotoxic effects or histological changes in the dura mater of the animals studied. RESUMO Objetivo Avaliar os efeitos da terapia fotodinâmica (PDT) na dura-máter usando os fotossensibilizadores cloreto de alumínio ftalocianina e azul de metileno em ensaios in vivo. Métodos Cinquenta e seis ratos Wistar machos divididos em dois grupos; um submetido à PDT e o outro submetido aos fotossensibilizadores sem a fotoativação (controle). Os fotossensibilizadores foram aplicados sobre a dura-máter depois de laminectomia no nível T10. Os métodos de avaliação usados foram a escala de avaliação funcional de Basso, Beattie e Bresnahan (BBB) e o estudo da dura-máter por microscopia óptica. Resultados Não foram observadas alterações da atividade motora dos animais submetidos à PDT com relação ao grupo controle. A avaliação histológica e histopatológica não mostrou diferenças entre o grupo exposto aos fotossensibilizadores ativados e o grupo controle, com relação ao processo inflamatório e à necrose tecidual. Conclusões O uso conjunto de PDT e os fármacos fotossensibilizantes cloreto de alumínio ftalocianina e azul de metileno não induziu efeitos neurotóxicos clínicos e/ou alterações histológicas sobre a dura-máter dos animais estudados. Los fotosensibilizadores se aplicaron a la duramadre después de la laminectomía en el nivel T10. Los métodos de evaluación utilizados fueron la escala de evaluación funcional de Basso, Beattie y Bresnahan (BBB) y el estudio de la duramadre mediante microscopía óptica. Resultados No hubo cambios en la actividad motora de los animales sometidos a PDT en relación con el grupo de control. La evaluación histológica e histopatológica no mostró diferencias entre el grupo expuesto a fotosensibilizadores activados y el grupo de control con respecto al proceso inflamatorio y la necrosis tisular. Conclusiones El uso conjunto de PDT con las sustancias fotosensibilizadores ftalocianina de aluminio clorada y azul de metileno no indujo efectos neurotóxicos clínicos o cambios histológicos en la duramadre de los animales estudiados. Abstract Objective: This study aims to investigate the incidence of postnatal diagnosis of congenital heart disease (CHD) and the predictive factors for hospital mortality. Methods: This retrospective cohort study was conducted at a Brazilian tertiary center, and data were collected from medical records with inclusion criteria defined as any newborn with CHD diagnosed in the postnatal period delivered between 2015 and 2017. Univariate and multivariate analyses were performed to determine the potential risk factors for mortality. Results: During the 3-year period, 119 (5.3%) children of the 2215 children delivered at our institution were diagnosed with CHD. We considered birth weight (P=0.005), 1st min Apgar score (P=0.001), and CHD complexity (P=0.013) as independent risk factors for in-hospital mortality. The most common CHD was ventricular septal defect. Indeed, 60.5% cases were considered as "complex" or "significant" CHDs. Heart surgeries were performed on 38.9% children, 15 of whom had "complex" or "significant" CHD. A mortality rate of 42% was observed in this cohort, with 28% occurring within the initial 24 h after delivery and 38% occurring in patients admitted for heart surgery. Conclusion: The postnatal incidence of CHD at our service was 5.3%. Low 1st min Apgar score, low birth weight, and CHD complexity were the independent factors that affected the hospital outcome.

Abstract Objectives: Hemoglobin SC is the second most common variant of sickle-cell disease worldwide, after hemoglobin SS. The objectives of the study were to describe the clinical and laboratory characteristics of hemoglobin SC disease in children from a newborn screening program and treated at a blood center. Methodology: This study assessed a cohort of 461 infants born between 01/01/1999 and 12/31/2012 and followed-up until 12/31/2014. Clinical events were expressed as rates for 100 patient-years, with 95% confidence intervals. Kaplan-Meier survival curves were created. Results: The median age of patients was 9.2 years; 47.5% were female. Mean values of blood tests were: hemoglobin, 10.5 g/dL; reticulocytes, 3.4%; white blood cells, 11.24 × 109/L; platelets, 337.1 × 109/L; and fetal hemoglobin, 6.3%. Clinical events: acute splenic sequestration in 14.8%, blood transfusion 23.4%, overt stroke in 0.2%. The incidence of painful vaso-occlusive episodes was 51 (48.9-53.4) per 100 patient-years and that of infections, 62.2 episodes (59.8-64.8) per 100 patient-years. Transcranial Doppler ultrasonography (n = 71) was normal given the current reference values for SS patients. Hydroxyurea was given to ten children, all of whom improvement of painful crises. Retinopathy was observed in 20.3% of 59 children who underwent ophthalmoscopy. Avascular necrosis was detected in seven of 12 patients evaluated, predominantly in the left femur. Echocardiogram compatible with pulmonary hypertension was recorded in 4.6% of 130 children, with an estimated average systolic pulmonary artery pressure of 33.5 mmHg. The mortality rate from all causes was 4.3%. Conclusions: Clinical severity is variable in SC hemoglobinopathy. Several children have severe manifestations similar to those with SS disease.


Resumo Objetivos: A hemoglobinopatia SC é a segunda variante mais comum da doença falciforme no mundo, após a hemoglobinopatia SS. Os objetivos do estudo foram descrever as características clínicas e laboratoriais da hemoglobinopatia SC em recém-nascidos diagnosticados por programa de triagem neonatal e encaminhados para acompanhamento em hemocentro. Metodologia: Coorte de 461 recém-nascidos SC nascidos entre 01/01/1999 e 31/12/2012 e seguidos até 31/12/2014. A incidência de eventos clínicos foi expressa por taxas relativas a 100 pacientes-ano, com limites de confiança a 95%. Curvas de sobrevida foram construídas segundo Kaplan-Meier. Resultados: Mediana de idade, 9,2 anos; 47,5%, feminino. Médias dos valores hematológicos: hemoglobina 10,5 g/dL; reticulócitos 3,4%; leucometria 11,24 x 109/L; plaquetometria 337,1x109/L; hemoglobina fetal 6,3%. Eventos clínicos: sequestro esplênico agudo em 14,8%, hemotransfusão 23,4%, AVC isquêmico 0,2%. A incidência de episódios vaso-oclusivos dolorosos foi de 51 (48,9-53,4) por 100 pacientes-ano; a de infecções, 62,2 episódios (59,8-64,8) por 100 pacientes-ano. Doppler transcraniano (n = 71) foi normal, se usados os valores de referência de crianças SS. Dez pacientes usaram hidroxiureia, todos com melhoria das crises dolorosas. Retinopatia foi observada em 20,3% das 59 crianças que fizeram fundoscopia. Necrose avascular foi detectada em 7 de 12 pacientes avaliados, com predomínio no fêmur esquerdo. Ecocardiograma compatível com hipertensão pulmonar foi registrado em 4,6% de 130 crianças, com média estimada de 33,5 mm Hg de pressão arterial pulmonar. A taxa de mortalidade por todas as causas foi de 4,3%. Conclusões: A hemoglobinopatia SC tem gravidade variável; várias crianças apresentam manifestações clínicas intensas, semelhantes às da hemoglobinopatia SS.

ABSTRACT Objective: This research study aimed at evaluating the inhibitory activity of Matricaria recutira (chamomile) hydroalcoholic extract on Candida albicans and Enterobacter cloacae biofilms. Methods: C. albicans and E. cloacae biofilms with thirty-hour formation were submitted, for five minutes, to 100, 200 and 300 mg / mL of M. recutita hydroalcoholic extract, chlorhexidine digluconate 0.12% (Periogard® - inhibition control) or sterile distilled water (growth control). Subsequently, they were washed and divided into two groups to determine the microbial viability: G/UFC - counting of colony forming units (cfu) in agar and G/DNA - quantification of viable DNA with violet crystal dye by spectrophotometry. Results: M. recutita extract at 300 mg/mL reduced significantly (p <0.01) the E. cloacae cfu/mL number in biofilm with results similar to chlorhexidine 0.12%, while extracts at 100 and 200 mg/mL did not have the same effectiveness. The amount of E. cloacae viable DNA was reduced (p <0.05) in all the M. recutita extract concentrations and chlorhexidine. There was no significant difference (p = 0.565) in the cfu/mL number or in the amount of viable DNA (p = 0.8094) in C. albicans biofilm when compared to untreated biofilm (control) or, even, between the extracts when compared to each other or to chlorhexidine 0.12%. Conclusion: 300 mg/mL M. recutita extract reduced significantly the E. cloacae biofilm but not the C. albicans, both with a similar result to chlorhexidine 0.12% (Periogar®).


RESUMO Objetivo: Avaliar a atividade inibitória do extrato hidroalcoólico de Matricaria recutira (camomila) sobre biofilme de Candida albicans e Enterobacter cloacae. Métodos: Biofilmes de C. albicans e E. cloacae com trinta horas de formação foram submetidos por cinco minutos a 100, 200 e 300 mg/mL de extrato hidroalcoólico de M. recutita, digluconato de clorexidina 0,12% (Periogard® - controle de inibição) ou água destilada esterilizada (controle do crescimento). Depois foram lavados e divididos em dois grupos para determinação da viabilidade microbiana: G1 - contagem de unidades formadoras de colônia (ufc) em ágar e G2 - quantificação de DNA viável com corante cristal violeta por espectrofotometria. Resultados: O extrato de M. recutita a 300 mg/mL reduziu significativamente (p < 0,01) o número de ufc/mL de E. cloacae em biofilme com resultados semelhantes a clorexidina 0,12%, enquanto os extratos a 100 e 200 mg/mL não tiveram a mesma efetividade. Já a quantidade de DNA viável de E. cloacae foi reduzida (p < 0,05) em todas as concentrações do extrato de M. recutita testadas e clorexidina. Não houve diferença significativa (p=0,565) no número de ufc/mL ou na quantidade de DNA viável (p=0,8094) no biofilme de C. albicans quando comparado ao biofilme sem tratamento (controle) ou mesmo entre as concentrações do extrato quando comparados entre si ou com a clorexidina 0,12%. Conclusão: O extrato de M. recutita 300 mg/mL reduziu significativamente o biofilme de E. cloacae mas não de C. albicans, ambos com resultado semelhante à clorexidina 0,12% (Periogar®).
